Description


Focus on improving provider prescribing practices by using ASB 201 data and pharmacy supported best practice guidelines.

We are looking to engage a physician champion, Executive leadership, and Pharmacy leadership.

While our focus is on provider prescribing practices, we would still like to engage Nursing team members through education to support provider decisions.

Educating clinicians about resistance and optimal prescribing.

Dedicating necessary human, financial and information technology resources.

Reporting

Regular reporting information on antibiotic use and resistance to doctors, nurses and relevant staff.

Tracking

Monitoring antibiotic prescribing and resistance patterns.

PDSA Cycles


Progress on PDSA Cycle 1

  1. Report and track provider prescribing data for ASB
    • Begin to use AU data to demonstrate ATX prescribing patterns for providers
  2. Engage physician champion
    • Identified another potential physician champion. Currently in process of working with Executive Director of Physician Services to solidify provider engagement moving forward.
  3. Engage Executive Leadership
    • Will reach out to Executive Director of Clinical Services for support and buy-in.
    • Inquire about possibility of implementing care team bedside rounding (Including MD & Pharmacy)
  4. Engage Pharmacy Leadership
  5. Educate physicians with best practice prescribing guidelines (Pharmacy)
    • Utilize the UW Pocket Guide and information presented in TASP sessions
  6. Present prescribing data and best-practice guidelines at Medical Staff meeting via Physician or Pharmacy champion
    • Dependant on finding physician champion and end of data collection cycle. This is our end goal for the 201 cohort.

Barriers:

1. Ongoing issues with discrete data not being readily available in our EHR hosted environment.

2 & 6. Provider that we identified as being a “potential” physician champion has declined our offer of engagement

4 & 5. Pharmacy is currently engaged with high-priority project: Oncology and hazardous compounding
