By September 1st Medical Staff, LHD3 Pharmacy will Develop a Vancomycin Dosing and Monitoring CDTA to present and gain approval for. This CDTA that will follow current updated guidelines and utilize AUC and troughs to monitor therapy.

Change: Assess Vancomycin Dosing Calculator: What on-line free calculator should be utilized by pharmacy staff? We will test which one is most user friendly and provides the appropriate trough range for pharmacy to adjust.
Who: Merilla & Jason will utilize past vancomycin data to test various availalbe calculators during the month of June to determine which one is their preference.
What will be required to prepare: Identify calculators that are available to test.
How to measure: Merilla will develop a spreadsheet to record various quantitave data as we are testing the various calculators so we can make an educated assessment of calculator preference.
