Description


  • That antibiotic prescribing is following the four moments of Antibiotic Desision Making:
    1. Make the diagnosis
    2. Cultures and empiric therapy
    3. Stop, narrow, change to PO
    4. Duration
  • Reminder for clinical pharmacists to assess the appropiateness of ordered antibiotics related to spectrum, duration and to encourage IV to PO changes 48 hours after antibiotics have been started.
