Progress on PDSA Cycle 1

Nov 1. Shauna P’s start date at Blue Mountain and transition data collection from Maisie to Shauna.

  1. SP on point for all infection prevention at BM (and EVS, Employee Health)
  2. Continue to collect and submit data to redcap survey
  1. Observe processes for collection
  2. Tracked age of patient, if abx were prescribed and duration of abx therapy
  3. Meeting scheduled in March 2023 with nurse educator to plan for mitigation plan
  4. Collected urine samples in clinic, inpatient, and ER
  5. Recorded the collecting RN, MA, and lab techs
  6. Recorded the lab tech who processed the samples
  1. Identified contamination issue with urine cultures (27% are contaminated). Presented findings to med staff in March 7, 2023
  2. Approx. half treated with abx.
  3. Abx prescribed before urine sensitivity data . Culture and sensitivity
  4. Will review prescribers of abx for ASB
  1. Shared data with clinic manager, lab manager, med staff meetings (clinic, inpatient, and ED)
  2. Personnel includes 6 MAs, 6RNs, 4 lab techs, 5 physicians/APPs, and locums/residents
  3. Making clinical connection for patients who require treatment when culture data are delayed
