Progress on PDSA Cycle 1

  • Shanna – IP – pulled away as main campus ASP meets at same time
  • Mia planning to focus on ED, Shanna on in-patient
    • Trying to get education to hospitalist group using baseline data 
    • Mia reaching out to hospitalist leadership to meet with them
    • Will continue to reach out
  • Continue working on diagnostic stewardship
  • Talked with IP, regulatory leaders
    • not only pharmacy, multi-disciplinary
    • Work with regulatory to set up meeting – after holidays, set up kickoff meeting (second week of Jan)
    • Get more people involved. – sharing workload
    • Understand work, know importance and can speak to others
    • No provider yet – try to identify someone and have multiple people connect
  • Modified restricted formulary implementation
  • Worked with IT to send weekly report on urine cultures
  • Met with new IP – not fully at West Valley
  • Order UA – does not automatically reflex to culture (pop-up) – BPA
  • May not use Redcap – currently entering data on an Excel sheet
